Siskiyou Community Health Care

An Affordable Solution For Quality Health Care

Siskiyou Community Health Center continues to build healthy communities by providing a wide range of affordable health services. Increase in demand for these services prompted us to move to a brand new Grants Pass facility in 2012. The new medical center houses our medical, mental health, Outreach, and laboratory services, as well as a pharmacy and a walk-in clinic. The walk-in clinic provides families a low cost alternative to going to the emergency room.

Soon, we will be expanding our Cave Junction medical facility. It will add a dental clinic, expand our medical and behavioral health departments and bring our Outreach on-site. The expansion will double our existing clinic size.

MISSION

To identify and provide care for primary health needs of our community
in a professional and compassionate manner.

VALUES

In compassion, honesty, and integrity guiding us in all of our actions.

In our commitment to provide the highest quality health care and customer service.

In treating our patients, clients and each other with dignity and respect, regardless of
race, age, gender, ability, values, lifestyle, or economic status.

That teamwork and cooperation are vital to the success of our working and community relationships.

That our success is measured only by how well we serve.
